Hacienda Skilled Nursing Facility is the only facility of its kind in the Southwestern United States, providing comprehensive, individual, home-away-from-home care to children and young adults (ages 45 or younger). We provide round-the-clock long-term, short-term, respite and palliative care tailored to meet their needs.
We understand that caring for an individual with profound disabilities and complex medical needs can be an emotional and difficult experience for a family.
At Hacienda Skilled Nursing Facility, we provide each patient and his family with a nurturing environment where they can heal, learn and grow.
Each patient’s care plan goals are based on his unique medical treatment needs. Our exceptional interdisciplinary team ensures that every individual receives the highest level of medical care and therapy that supports his overall well being and enhances his quality of life.
We believe that everyone deserves to live a full life. Many of our young people attend local public schools, while others receive individualized education on our campus. Each patient has the opportunity to participate in therapeutic recreational activities such as arts and crafts, music, singing and field trips.
Parents and caregivers are an integral part of a patient’s care plan, and our goal is to ensure that when a child, or young adult returns home, the parent or caregiver has received the education, training and support that he needs to enhance the individual’s well being at home.
We believe that all families should receive the care that they deserve, not the care that they can afford. Your support helps us to bridge the gap between Medicaid and the true cost of this specialized care. To learn how you can support our work and make a difference in someone’s life, visit Ways to Give.
Hacienda Skilled Nursing Facility also manages two on-campus apartments. The apartments provide a comfortable place for out-of-town parents or caregivers to stay so they can be close to their loved ones while visiting or while training in the transition-to-home program.
